Click the link to the folder which contains the form you need and then click on the appropriate form in the folder to begin the download. General Forms
The Responsibility Release & Transportation Information and Final Count/Awards forms need to be returned, one per school/organization per performance day. Please only fill out and return the appropriate rooming list form if your group is staying overnight coordinated through Music USA Festivals.
Performing Group Specific Forms
These forms need to be filled out, one for each separate performing group.
